The Evaluation Process
Overview Process

The first step in the evaluation process is to set up appointments for the Parent Interview, Evaluation, and Follow-Up Conference. Once these appointments have been scheduled, you will receive an information packet in the mail.

Two Weeks Prior to Evaluation Day
• Please complete and return the forms enclosed in the information packet. These include the Social History Form regarding your child's social, medical, educational, and family history and "Release of Information" and "Consent to Evaluate" forms.
• If available, please send previous testing and/or assessments. Work samples can also be helpful in the evaluation process.
• Include a non-refundable deposit of $300.00 (this is applied towards the evaluation fee).
• Be sure that your child has had both a recent hearing and vision screening.

One Week Prior to Evaluation Day
• If forms and deposit have not been received by the Diagnostic Center, the evaluation will be canceled or rescheduled.

Parent Interview
Before the first evaluation day, parents are invited to meet with the psychologist who will be completing their child's testing for an initial interview. At this meeting, the psychologist will talk with parents about their reasons for pursuing testing and will gather additional history and information about the child. This is also an opportunity for parents to ask any questions about the evaluation process and to address any related concerns.

Evaluation Day
• The remainder of the fee is due on the first day of the evaluation.
• Testing is typically conducted over two sessions on separate days. Testing times will be established when you set up your child's appointment.
• If your child takes medication on a regular school day please do not take a break for the evaluation - medication should be taken in the way that your child takes it on a typical school day.
• If they wear glasses, make sure that they have them with them.
• Please dress them as though it is a regular school day.
• We usually offer a break and a snack to children during evaluation sessions. If your child would prefer a specific snack, we ask that you bring that with you when you come to drop them off. You may stay in the Center while your child is testing if you choose but are not required to do so.

After Evaluation Day
• After all of the data has been gathered and analyzed, results and recommendations are shared via a reporting conference, and families are provided with a comprehensive written report. Your psychologist will work with you to schedule this conference. This usually occurs within six weeks of testing, if not sooner. If you feel it would be beneficial, please feel free to invite other individuals involved with your child to this meeting.
• Any changes needed are made to the written report, and it is sent to the parents.
• Understanding that questions arise after the evaluation and reporting conference, the Center provides ongoing communication as needed; however, additional reporting conferences will incur charges as consultation services.

Consultation Services

Families often come to The Diagnostic Center for a review of existing testing reports or other consultations. The consultation fee is $125 per hour. Examples of consultative services include the following:

  • Review of tests and assessments not given at the Center
  • Referrals or recommendations given based on the review
  • Consultation with parents, professionals, and school personnel following review or evaluation
  • Conferences following evaluations in the Diagnostic Center in addition to the initial reporting conference
  • Observations of students in their school environment
