NEW STUDENT/FAMILY FORMS

 
1. Click here to Login to MY CIA.

2. After logging in, you will see a pale yellow banner with the number of forms that need to be filled out. Most families will have 10 online forms to complete. (Note: Here are step-by-step instructions about logging into the website.)
 
3. All forms need to be completed by August 7. **For families with two households, both parents/guardians of that student will need to sign all forms.**
 
4. Two optional medical forms below require a doctor's signature. Please download, print and return with doctor's signature to the school if it is applicable to your child:
 
5. Two athletic forms need to be downloaded, printed and returned to school if your child will participate in athletics (K-12). 
Athletics Medical Form (physician signed)
Athletic Concussion Form (signed by student and parent)

6. Immunization forms are needed for all children starting kindergarten, 7th grade or a new student starting school in Tennessee for the first time. Under Tennessee State law, all children starting kindergarten, seventh grade, or as a new student starting school in Tennessee for the first time need to provide the school a current official Tennessee Certificate of Immunization prior to their first day of attendance.
